Executive Summary

Edge infrastructure and web frameworks dominated the last 48 hours. A three-zero-day Ivanti Connect Secure chain gives attackers root on fully patched VPN appliances without leaving malware, and Cisco confirmed a second actively exploited Secure Email Gateway zero-day in a month. Vercel shipped an out-of-band Next.js patch for two critical unauthenticated RCEs. On governance, the UK AISI published eight frontier AI best-practice priorities; a reported listening device at OpenAI raises physical and insider risk at model providers.

Ivanti Connect Secure Three-Zero-Day Chain: Rootkit-Free Persistence on Patched VPNs

CRITICAL URGENCY

Summary: Volexity reports that UTA0576 chained an authentication bypass, an RCE (CVE-2026-104935) and a privilege escalation against Connect Secure 22.8R2.4 since October 3. The actor re-exploited for each command rather than installing persistence, and tampered with logs to evade the Integrity Checker Tool. Post-exploitation focused on Active Directory secrets, LAPS passwords and BitLocker recovery keys. The privilege-escalation fix is reportedly still pending.

Key Sources:

Why This Matters: A clean integrity check no longer proves a clean appliance. Assume credential exposure for AD tiers reachable from the VPN and prioritise compromise assessment and credential rotation over patching alone.

Cisco Secure Email Gateway Second Zero-Day: Attachment Path Traversal to Root (CVE-2026-76519)

HIGH URGENCY

Summary: Cisco PSIRT confirmed active exploitation of a path traversal in the Advanced Malware Protection handling of email attachments. A crafted attachment allows arbitrary file writes and root command execution when File Reputation is enabled and Content Scanner Tools is older than 25.3.0.5525. It follows the September SQL-injection flaw (CVE-2026-76461).

Key Sources:

Why This Matters: Repeat exploitation of one gateway class argues for compromise assessment of mail-security appliances, not just patching, and for scrutiny of any inline scanner, including AI-assisted email triage, that parses hostile input.

Next.js Out-of-Band Patch: Critical RCE in use cache and librsvg

HIGH URGENCY

Summary: Vercel released Next.js 16.4.1 and 15.5.28 to fix two unauthenticated RCEs. One affects use cache functions returning typed arrays with Cache Components and has no workaround. The other is a memory flaw in librsvg reached through the Image Optimization API. Next.js fronts many AI chat and agent applications, so exposure is broad.

Key Sources:

Why This Matters: Upgrade is the only remedy for the caching flaw. The librsvg issue also highlights transitive native dependencies as an RCE surface in AI-application front ends.

UK AISI’s Eight Priorities for Frontier AI Technical Best Practice

MEDIUM URGENCY

Summary: On October 12 AISI published the technical practices it considers most urgent for frontier developers: documented risk assessment, lifecycle controls and monitoring, preserving oversight as capability grows, defining and reporting unsafe agent behaviour, sharing progress metrics, and qualified independent auditors. It is explicitly voluntary, so the open question is how it will feed enterprise vendor due diligence.

Key Sources:

Why This Matters: Expect these priorities to surface in AI vendor questionnaires. Agent-incident reporting expectations may interact with your own customer incident-notification obligations.

When the Frontier Lab Is the Target: OpenAI Listening Device and Concentrated Risk

MEDIUM URGENCY

Summary: A small encrypted recording device was found on September 30 in a USB port behind a TV in an OpenAI executive conference room, in place since September 28. An employee was arrested October 9 and charged with illegally intercepting oral communications. OpenAI says it found no access to weights, code or customer data; the investigation is ongoing. Details come from reporting on the charging complaint and are allegations.

Key Sources:

Why This Matters: A few frontier labs hold strategic IP and critical functions for customers. Physical access, insiders and executive communications at model providers are a third-party concentration risk worth adding to due diligence.
