Executive Summary

Edge infrastructure and web frameworks dominated the last 48 hours. Volexity disclosed a three-zero-day Ivanti Connect Secure chain that gives root on fully patched appliances without malware. Cisco confirmed a second exploited root flaw in Secure Email Gateway within a month, and Vercel issued an out-of-band fix for two critical Next.js RCEs. On governance, the UK AISI published eight frontier-AI best-practice priorities. Assume credential exposure on Ivanti gateways and patch Cisco and Next.js now.

Ivanti Connect Secure Three-Zero-Day Chain: Rootkit-Free Persistence on Patched VPNs

CRITICAL URGENCY

Summary: Volexity attributes the campaign to UTA0576, which chained an authentication bypass, an RCE (CVE-2026-104935) and a privilege escalation to run root commands on Connect Secure 22.8R2.4 since October 3. The actor deployed no malware or web shell, re-exploited for each command, and tampered with logs to evade the Integrity Checker Tool. Targets included Active Directory secrets, LAPS passwords and BitLocker recovery keys. The privilege-escalation fix is reportedly still pending.

Key Sources:

Why This Matters: A clean integrity check no longer proves a VPN gateway is uncompromised. Assume credential exposure, rotate AD-tier secrets, and apply mitigations now.

Cisco Secure Email Gateway Second Zero-Day: Attachment Path Traversal to Root (CVE-2026-76519)

HIGH URGENCY

Summary: Cisco PSIRT confirmed attackers are exploiting a path traversal in SEG Advanced Malware Protection attachment handling. A crafted attachment gives arbitrary file writes and root command execution when File Reputation is enabled and Content Scanner Tools is older than 25.3.0.5525. It follows the September SQL-injection root flaw (CVE-2026-76461).

Key Sources:

Why This Matters: Two exploited root flaws in one product class signal sustained attacker focus on inline scanners that process attacker-controlled content. Patch, then run a compromise assessment on gateways.

Next.js Out-of-Band Patch: Critical RCE in use cache and librsvg SVG Processing

HIGH URGENCY

Summary: Vercel released Next.js 16.4.1 and 15.5.28 on October 14 to fix two unauthenticated RCEs. One affects use cache functions returning typed arrays with Cache Components, with no workaround. The other is a memory flaw in librsvg reached through the Image Optimization API. Next.js is the default front end for many AI chat and agent applications, so exposure is broad.

Key Sources:

Why This Matters: Because one flaw has no workaround, upgrading is the only fix. The case also shows transitive native dependencies as an RCE path in AI application front ends.

UK AISI’s Eight Priorities for Frontier AI Technical Best Practice

MEDIUM URGENCY

Summary: On October 12 AISI published the technical practices it says frontier developers most urgently need: documented risk assessment, lifecycle controls and monitoring, preserving oversight as capability grows, defining and reporting unsafe agent behaviour, sharing progress metrics, and qualified independent auditors. It is voluntary and standards-oriented, so the strategic question for enterprises is its effect on vendor due diligence and procurement.

Key Sources:

Why This Matters: Expect these priorities to surface in model-provider questionnaires. Agent-incident reporting rules may also interact with your own incident-notification obligations.

When the Frontier Lab Is the Target: OpenAI Listening Device and Concentrated Insider and Physical Risk

MEDIUM URGENCY

Summary: A small encrypted recording device was found on September 30 in a USB port behind a TV in an OpenAI executive conference room. An employee was arrested October 9 and charged with illegally intercepting oral communications. OpenAI says it found no sign of access to weights, code or customer data. The details come from charging-complaint reporting and should be treated as allegations.

Key Sources:

Why This Matters: A handful of labs hold strategic IP and critical customer dependencies. Physical access, insiders and executive communications are a concentration risk for downstream enterprises, so ask model providers about their personnel and physical security.
